We are seeking an enthusiastic and suitably qualified worker for a new case management role within the Mental Health Program (MHP). This position will provide recovery oriented case management services to clients exiting the New Horizons program (with preference to clients exiting Intensive HBOS and SECU Diversion).

We are seeking suitably qualified and experienced Podiatrists to provide podiatry services to inpatients, aged care residents and primary care clients, and outreach services to HACC clients at Rushworth and Stanhope. The successful applicants will be able to work autonomously and as part of a multi-disciplinary team and be eligible for membership of the Australian Podiatry Association.

The role is responsible for the delivery of quality occupational therapy services to inpatients, aged care residents, primary care (non-HACC) and TCP clients of Rochester and Elmore District Health Service. The Occupational Therapist will work as part of a multidisciplinary team, and liaise with health care providers to provide appropriate assessment, referral and care, and promote awareness of occupational therapy to staff, consumers and the community.

The TCP Manager coordinates and supports the TCP team and coordinates referrals, assessments and allocations in collaboration with inpatient units, multidisciplinary teams and ACAS. To be successful in this position, you must be a Registered Nurse or Allied Health Practitioner with relevant registration, and hold appropriate post graduate qualifications and/or experience within a Community / Primary Health management role or similar.

This community based role, for a Registered Nurse, Division 1, will provide expert clinical care, advice and support enabling effective palliative care to clients, their family and carers within the local district. The position could offer an ideal opportunity for a Nurse Practitioner Candidate.

Hours of work will be negotiated with two (2) successful applicants to ensure continuity of service between 8.30am and 5.00pm Monday to Friday.

We seek a suitably qualified and experienced psychiatrist with Fellowship of the RANZCP or evidence of significant progress towards achievement of same. The position includes some regional travel and on call duties are required. Immigration and Visa assistance is available if required.

A full time, temporary five (5) month position currently exists in our Warrnambool Community Adult Mental Health Team.
